The online application allows you to attach PDF documents. Please do not mail, email, or fax documents. If you begin the application process, but do not submit the application, the information will be saved for only five (5) days. After your information is complete, you will receive a confirmation number, and your information will be saved allowing you to return at any time to submit additional applications.
